Care Navigation Partnership West Essex

West Essex Care Navigation Partnership provides a free and impartial service to anyone in West Essex (over the age of 16) looking for help with things that might be affecting their health and wellbeing. 

Care Navigation offers free and confidential connections to support with the following:

  • Healthy Living
  • Reducing social isolation
  • Managing long term conditions
  • Increasing personal safety & security
  • Mobility & equipment
  • Looking after someone

The West Essex Care Navigation is a scheme commissioned by West Essex Clinical Commission Group (CCG), Public Health and Essex County Council and a partnership with Age UK Essex, Smart Life, Community Agents Essex and Provide Lifestyle Services. It builds on the work already undertaken by the Smart Life project to increase the role of social prescribing in GP Practices.
